Breaking down barriers for housing!

Housing Transition

Services that help a person plan for, find, and move to a home in the community include:

Covered Services:

1. Developing a housing transition plan.
2. Supporting the person in applying for benefits to afford their housing, including determining eligibility for available benefits.
3. Assisting with the housing search and application process.
4. Aiding in tenant screening and housing assessments.
5. Providing transportation while discussing housing-related issues with the person receiving services.
6. Helping the person understand and create a budget 7. Promoting/supporting cultural practice needs and understandings with prospective landlords, property managers* 8.Helping the person find funding for deposits*

Not Covered Services:

1. Room and Board (including moving expenses).
2. Deposits.
3. Food.
4. Rent.
5. No-show appointments.
6.Staff travel time with a person not in the vehicle or the person is in the vehicle but Housing Stabilization Services is not discussed.
7. Direct or indirect services provision in a group setting.
8. Transition services from institutions if a person dies before discharge or transition services exceed 180 days.

Housing Sustaining

Services that assist a person in planning for, finding, and moving to a home in the community include:

Covered Services:

1. Developing, updating, and modifying the housing support and crisis/safety plan on a regular basis
2. Preventing and early identification of behaviors that may jeopardize continued housing
3. Educating and training on the roles, rights, and responsibilities of both tenants and property managers
4. Providing transportation while discussing housing-related issues with the person receiving services
5. Supporting and promoting cultural practice needs and understandings with landlords, property managers, and neighbors
6. Coaching to develop and maintain key relationships with property managers and neighbors

Not Covered Services:

1. Room and board (including moving expenses).
2. Deposits.
3. Food.
4. Furnishings.
5. Rent.
6. Utilities.
7. No-show appointments.
8. Staff travel time when the person is not in the vehicle.
9. Direct or indirect services provided in a group setting.
10. Services provided outside of Minnesota.
